The MetroHealth Community Health Center is looking for an APP to support our growing programs. We are seeking a provider who can provide care to patients across the lifespan with primary locations being in Collinwood and Euclid. Community Outreach within the FQHC-LAL continues to grow, and your clinical location may change daily (schedule known in advance) at a community partner clinic or a mobile unit. This APP will serve patients across the lifespan (pediatric through geriatric), offering comprehensive primary care, telehealth services, chronic disease management, and care coordination with our integrated team. Family Medicine certification and experience required.
1 year of APRN experience required.
2 years of APRN experience preferred.
