Biweekly Hours: 80.00
Shift: 8:00 - 4:30

Summary:
Coordinates and provides oversight for assigned School Health Program (SHP) region and population health initiative(s), including serving as a charge nurse at assigned schools. Performs clinical Registered Nurse (RN) functions (e.g. providing immunization assistance) as assigned by appropriate members of the School Health Program medical team. Works independently on a variety of functions to support the SHP. Assists with communication, data collection, and outreach activities. Manages relationships with internal and external partners and stakeholders. Plans and coordinates clinical activities impacting patient care delivery; ensures appropriate care is given and provides guidance as needed. Maintains accountability for ongoing SHP region operations and population health initiatives, including practice and financial management. Collaborates with the SHP team on the provisioning of patient care, performing clinical reviews of selected patients to determine service needs, and providing case management/coordinating the care of patients enrolled in the SHP.
Qualifications:
Required: Bachelor of Science degree in Nursing (BSN) from an accredited school of nursing. Possess a current Registered Nurse (RN) license for the State of Ohio. Current BLS certification. One year of experience in nursing, including one year of experience caring for school-age children. Ability to work independently and as a member of a multidisciplinary team. Excellent written, verbal, and interpersonal communication skills. Excellent organization, time management, and problem-solving skills. Strong analytical, data management, and computer skills. Access to reliable transportation to travel to clinical sites. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: Fluency in writing, speaking and interpreting both English and Spanish. Experience with case management, access management, or similarly related experience. Knowledge of EPIC. Strong leadership skills Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. May need to lift up to twenty-five (25) pounds on occasion. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents.
